1. ASSUMPTION OF RISK

I acknowledge that participation in the NHA Region 3 Aircrew Challenge (the “Event”) is a physically demanding activity that includes, but is not limited to, running approximately five (5) miles and completing obstacle stations such as bodyweight exercises, weighted carries, and water-based evolutions including swimming and buddy tows.

I understand that participation involves inherent and significant risks, including but not limited to: slips, falls, drowning, collisions, heat-related illness, dehydration, musculoskeletal injuries, cardiac events, environmental hazards, and other serious injury or death.

I voluntarily and knowingly assume all risks, both known and unknown, even if arising from the negligence of the Naval Helicopter Association (NHA) or others, and accept full responsibility for my participation.

2. CERTIFICATION OF FITNESS

I certify that I am physically and medically capable of safely participating in this Event and have not been advised otherwise by a qualified medical professional.

I agree to monitor my own condition and immediately discontinue participation if I experience injury, illness, or unsafe conditions.

3. WAIVER, RELEASE, AND INDEMNIFICATION

To the fullest extent permitted by law, I, on behalf of myself and my heirs, assigns, personal representatives, and next of kin, hereby fully and forever waive, release, discharge, and agree to indemnify and hold harmless the Naval Helicopter Association (NHA), its officers, directors, employees, agents, contractors, volunteers, sponsors, and affiliates (collectively, the “Released Parties”) from any and all claims, demands, damages, losses, liabilities, or causes of action of any kind arising out of or related to my participation in the Event.

4. U.S. GOVERNMENT NON-LIABILITY

I acknowledge that this Event is conducted on a United States military installation. I understand and agree that the United States Government, the Department of Defense, NHA, and the Department of the Navy are not responsible for, and shall not be liable for, any injury, loss, or damage suffered by me in connection with this Event.

I further acknowledge that the Event is privately organized and is not an official government activity, and that the U.S. Government does not endorse or assume liability for the Event.

5. BASE ACCESS AND COMPLIANCE

I agree to comply with all installation rules, regulations, security requirements, and lawful orders while on Naval Station Mayport.

I understand that:

Access to the installation is a privilege subject to base security requirements

I may be subject to identity verification, vehicle inspection, and search

Failure to comply with base regulations may result in removal from the installation and/or denial of participation.

6. MEDICAL TREATMENT AUTHORIZATION

I authorize NHA and its representatives to obtain or provide emergency medical treatment deemed necessary in the event of injury or illness.

I understand that:

I am solely responsible for all medical costs incurred

The Released Parties assume no liability for the provision of such care

7. RULES AND SAFETY COMPLIANCE

I agree to follow all Event rules and safety instructions. I understand that failure to comply may result in removal from the Event without compensation.

8. PUBLICITY RELEASE

I grant NHA and its affiliates the right to use my name, image, likeness, voice, and appearance in photographs, video, or other media for promotional or informational purposes without compensation, in perpetuity.

9. SEVERABILITY

If any provision of this agreement is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.
